FLU SHOT CLINIC
The Department of Health is holding a flu shot clinic for Spearfish students and the general public
on Thursday, November 7 from 3:30 – 6:30. The clinic will be held in the SHS Commons area – just as you enter from the parking lot. The Tdap vaccination will also be available for students from ages 11-14 who have not previously received one. Check your child’s immunization records. The vaccine consent forms and vaccine information statements are available on the Spearfish Middle School Website. Click the following link to access the forms: Flu Shot Information Link
Parents/guardians are encouraged to come to the clinic to be with their child. If you are unable to
attend with your older child, those 6th grade and older will be allowed to receive immunizations with a signed consent form without a parent present. Parents – this is a good opportunity for you to get a flu shot as well. There is no
charge for the flu shots.
